Teche Action Clinic is a Federally Qualified Community Health Center (FQHC) that has been in operation for close to 40 years.
The clinic serves an annual patient base of 17,280 throughout rural southwest Louisiana. Teche Action Clinic’s scope of services includes primary and preventative medical care (Internal Medicine, Family Practice, Pediatrics, Obstetrics/ Gynecology), Behavioral Health, Pharmacy, Laboratory, WIC and Dental Services.
Teche Action Clinic is led by CEO, Dr. Gary M. Wiltz, a well-known and established leader in the FQHC movement. As a believer in safe quality patient care, Dr. Wiltz assisted Teche Action Clinic in becoming the first FQHC in Louisiana to achieve Joint Commission Accreditation in 1999 and the first to achieve Primary Care Medical Home (PCMH) designation in 2012.
After an exhaustive evaluation process, Teche Action Clinic chose CGM ENTERPRISE EHR to complement the CGM ENTERPRISE PM that had already been in use as a practice management solution at all eight of the clinic’s sites for more than 10 years.
With such a broad array of services rendered to an equally diverse patient population, Teche Action Clinic sought an EHR-PM solution that would seamlessly integrate patient care delivery, documentation and reporting as well as patient selfcare management.

The clinic needed to ensure that this designation was not compromised. A Primary Care Medical Home is required to demonstrate that it delivers patient-centered, comprehensive, coordinated and accessible care that is committed to quality and safety.
CGM ENTERPRISE enabled Teche Action Clinic to achieve several, if not all of its objectives for implementing an EHR-PM system.
With the ability to provide patients a summary of medical information in electronic format, the clinic improved its emergency preparedness goals for both patients and the organization.
Notable among several efficiency-enhancing tools is the Pattern feature that is built into CGM ENTERPRISE EHR.
This feature allows the physicians at the cClinic to make a quick order set of best practices based on previous documentation, assessment and treatment processes.
